After the creation of 1.0 mm of space between the first molar and second bicuspid, bond brackets on all posterior teeth, ligature tie the first and second molar.

Step one:

  1. Insert a .018 NiTi or a .016 SS full arch wire if possible
  2. Place Bumper sleeve from the first bicuspid to the first bicuspid
  3. Anneal the end of the arch wire and place a crisp bend-back (cinch)
  4. Place a Separator and an Open coil spring between the first and the second bicuspid
  5. Place an Essix retainer from the cuspid to the cuspid, also covering the alveolar process

Step two (after one month):

  1. Ligature tie from the second molar to the second bicuspid
  2. Cut back the facial surface om the cuspids from the Essix retainer
  3. Bond bracket on the cuspids
  4. Place a .016 SS full arch wire, if possible
  5. Place a center bend between the bicuspids
  6. Place a Separator and an Open coil spring between the first bicuspid and the cuspid

Step three (two to three months later):

  1. Ligature tie from the second molar to the first bicuspid
  2. Discard the Essix retainer
  3. Place a .016 SS or a .018 SS full arch wire
  4. Place an off-center bend between the first bicuspid and the cuspid
  5. Place Separators between the cuspid and the lateral incisor
  6. Place an Open coil spring from the ipsilateral to the contralateral cuspid
  7. Bend back (cinch) the arch wire behind the second molar

Step four (two to three months later}:
